Gather the following information:
- Class number and section (i.e. MTH 1240-01)
- Class instructor (i.e. full name – Joe Leader)
- Date and time of the exam (i.e. 10/12/2024, start at 8:00 a.m.)

- Type/name of assessment (week 3 quiz; Unit 1 Test; midterm)
- What accommodations you will need (i.e. 1.5X extra time; reduced distraction; test read aloud; etc.)
- Students will need to complete the Testing Accommodations Request Form for each class. Students can schedule up to 5 exams/quizzes at a time per course. You are limited to 5 exams/quizzes per form, but can complete additional forms as needed. Complete the form at least four business days prior to the exam, particularly if accommodations include having the test/exam read aloud as this requires time to prepare or extended time falling outside of normal Testing Center hours to ensure we have staff available.
- If a faculty member assigns a test with less than four business days’ notice, reach out to BOTH Disability Services ds@mtc.edu and testing@mtc.edu with the situation and complete request form as soon as possible. All such requests will be confirmed with the faculty member that they assigned a test or quiz with less than four days’ notice before the appointment will be scheduled for the student.
Please note: We cannot guarantee your exam will be ready unless it is scheduled 4 business days in advance due to staff availability.
